Account Forms and Online Services

The Minnesota College Savings Plan offers two methods to perform account transactions. You can choose to transact online through our secure online access or print and mail completed forms.
All forms are in PDF format and require the free Adobe Acrobat Reader
Online Services
To transact online, you will need to establish secure online access. Once online account access has been established, you will be able to:
- Open a new account
- Make a contribution to your account
- Rebalance or change your investment allocation
- Take a withdrawal
- Set up an auto contribution plan
- Elect e-Delivery of statements and/or disclosure documents
Print and Mail Account Forms
Select any of our forms from the list below. Print and complete the forms and mail to the address below. Click "sample" link to view a sample completed form:
For New Accounts:
To open an account, read the Disclosure Booklet carefully, then select from one of these three account Applications:
- Individual Account (sample)
- Custodial Account for a Minor (UGMA/UTMA) (sample)
- Entity Account (sample)
For Existing Accounts:
- Set up banking information and/or an automatic contribution plan (sample)
- Make an additional contribution by mail (sample)
- Rollover or transfer from another account* (sample)
- Make a withdrawal (sample)
- Set up automatic payroll deduction (sample)
- Rebalance or transfer funds among investment portfolios (sample)
- Change address, phone number or contingent account owner (sample)
- Change your beneficiary (sample)
- Change your account owner (sample)
- Create a power of attorney
* Before doing a transfer or rollover from another account, be sure to consider the differences in features and costs, and possible tax consequences.
